The Challenge
Lady discharged from hospital end of life after catastrophic stroke with Hospice at home. In her own environment regained some strength. Nonverbal but indicated to family that “What Matters” to her is to sit out and watch TV. Family were lifting.
The Change
ICF OT installed hoisting equipment; citizen only wanted her family to hoist – training was provided for family and care was cancelled.
ICF OT obtained funding from Live at Home fund for a moveable bespoke chair in the colour of citizens choice.
The Impact
Huge positive impact on the citizens overall well-being and What Matters to her was fully achieved; she can now watch TV from her armchair and be more inclusive with family activities.
The family can safely transport citizen without fear of injury.
